Current browsers accept PNG and SVG favicons without trouble. ICO still has the advantage of packing several sizes into one file and working in very old browsers, which is why many sites still offer both.
Because you are enlarging an image that originally had very few pixels. The detail information was never in the file; scaling can only interpolate between the pixels that exist. For a large icon you have to start from the original design, not from the small version.
They are near-identical formats: .cur is the Windows cursor variant and adds the hotspot position, the exact pixel the pointer clicks with. Structurally they share practically everything else.
